cmd: add PKGPROXY_CACHEDIR env var for serve - #157

Resolve the cache directory in serve's PersistentPreRunE using flag -> env var -> default precedence, matching the PKGPROXY_HOST / PKGPROXY_TRUST_PROXY pattern. Explicit --cachedir always wins, including --cachedir cache; an empty PKGPROXY_CACHEDIR falls through to the default.
